Job Description

JFCS seeks a compassionate professional to work with children receiving behavioral-health services as they progress toward a more self-sufficient lifestyle and potential family reunification, while providing guidance and ongoing support to the families and caregivers of the youth receiving services.

Responsibilities: • Provide case management for children enrolled in the behavioral-health system • Maintain weekly client contact, including home/community visits and medication checks • Help youth navigate hospital discharge and find intensive services as clinically indicated • Coordinate with the Department of Child Safety, probation/parole, DDD, schools, and other providers • Coordinate and facilitate Child & Family Team (CFT) meetings • Complete referrals, prior authorizations, and health plan paperwork

Qualifications: • High school diploma/GED plus 4 years of behavioral health experience, OR an A.A. plus 2 years, OR a Bachelor's in social work, psychology, or a human services field • Level 1 Fingerprint Clearance Card, reliable transportation, AZ driver's license and auto insurance
