Develop and coordinate programs linking elderly residents, especially those who are frail or disabled, to the supportive services they need to continue living independently.
Responsibilities: • Provide information and referrals on local, state, and federal programs and resources • Interview and screen residents for program and entitlement eligibility, and assist with applications • Assess residents' physical, mental, social, and financial needs and develop service delivery plans • Establish links with agencies and service providers, including the county division on aging • Serve as residents' advocate and liaison, engaging family members as needed • Plan and implement monthly social, educational, and recreational programs • Develop volunteer programs and informal support networks
Qualifications: • Bachelor's degree in psychology, sociology, counseling, or a related field • Two years of supervisory experience in a related field
