The Elder Services Case Aide supports the Case Manager, Assistant Program Director, and Program Director in helping seniors access the services they need.
Responsibilities: • Assist with processing authorizations and resolving service delivery problems • Help evaluate client eligibility for programs, including Medicaid eligibility assistance • Complete accurate, timely documentation and maintain client records • Provide clerical support, track service logs and calendars, and complete required service reports • Communicate with potential clients for service evaluation and provide appropriate referrals
Qualifications: • High school diploma or equivalent • Considerable knowledge of community organizations and resources; Microsoft Office (Excel, Word, Outlook) • Detail-oriented with efficient, accurate record-keeping • Must complete 701b Assessment training and certification within 12 months • Valid driver's license with proof of auto insurance
