Louis Brier Home and Hospital (Vancouver), a 215-bed long-term care facility guided by Jewish traditions, seeks a Social Worker, reporting to the Manager, Allied Health Services.
Responsibilities: • Provide facility-wide professional social work — pre-admission data collection, admission support, and continuous support to residents and families • Provide adjustment counselling and ongoing support for social, emotional, economic, and environmental concerns • Act as liaison with the Continuing Care Program; champion Person and Family Centered-Care • Document in resident records; support quality-improvement initiatives and community collaboration
Qualifications: Master's or Bachelor's in Social Work from an accredited university (Gerontology coursework), registration with the BC College of Social Workers, and 2 years' recent related experience (Long Term Care/Geriatrics preferred); strong counselling and interpersonal skills; knowledge of Jewish Values and Traditions an asset.
Schedule: Mon–Fri 8am–4pm. Salary: $41.04–$51.27/hour.
